Survey Information for Midwives and Doctors

Here is a bit more information about the guide.

We invite you to provide us with information about your practice for our 2012 updated comprehensive birthing resource guide. As a provider of maternity care services, the work you do with women in the childbearing year is incredibly valuable. Our web-based guide will help women navigate the options available to them in the community regarding their prenatal, birth, and postpartum care.

As you are the expert on your own practice we are asking that you provide the information that we will include in our guide. We have developed a survey in order to obtain comparable facts about each practice. To access the survey, please click the button below with the link to the online form.

If you have multiple providers in your practice, we only ask for one completed survey for the entire practice. Our survey is detailed, but the more information you are able to include, the more helpful the resource will be and the more women and families will be able to understand the care you provide.

A similar survey will be sent to non-medical care providers associated with childbirth, such as doulas, massage therapists, childbirth educators and yoga instructors, so that they may also be included in the guide. Our hope is that by providing a single resource with all childbirth-related services, women may discover the rich network of care that exists in our area.
